How do I start?

Step 1.

Basic Demographic information

Insurance Information (if applicable) 

  • Health History Form
  • Lifestyle Pattern Quiz.  This will pinpoint your lifestyle patterns or habits that have been preventing you from losing weight or keeping it off.  The Quiz results will help us personalize and prioritize a weight management program to best met your needs.

Step 2.

Once we have gathered all of this information, you will be contacted within 24 hours to set up a meeting time either in person or online.   Your first visit will be approx.  1-1 1/2 hours in length with a Nurse Practitioner who is a Certified Diabetes educator,  Nutritionist and Health Coach.

During this visit they will obtain a detailed diet, activity and medical history.

If in person: a physical exam, weight, height, blood pressure and comprehensive foot exam will be done.

If online:  verbal weight, height and recent blood pressure readings.

Step 3.

Together, you’ll start designing a simple meal plan that fits your budget and satisfies your tastes to help in living better.  Visits will be based on your educational needs and budget.

Step 4

Weight loss class
My husband and I attend the weekly “Healthy Living” class.   I have lost over 30 lbs and my husband Ben has lost over  45 lbs.

You will decide what program best meets your lifestyle:

  • Intensive Behavioral Therapy Weekly Support Group
  • Individual weekly one on one meeting
  • Online weekly SKYPE or email correspondence.

COSTS FOR WEIGHT LOSS

INSURANCE

Medicare pays for weekly Intensive Behavioral  therapy for weight loss up to 22 visits per year.  Must have a BMI >30.  Other private insurances may pay as well.  We will need to call for benefits.

CASH PAY:  (NOT FILING WITH INSURANCE)

First initial visit:  $150 for 1 ½ hours

Intensive Behavioral Therapy Weekly Support Group  $50.00/month

Individual weekly one on one meeting $60.00/month

Online weekly SKYPE or email correspondence 60.00/month
